Insurance Coverage For Eye Exams

for all comprehensive vision examinations*

Alberta Health Care

Please ensure that you bring your Alberta Health Care card with you to your initial eye exam. We will keep your Alberta Health Care number on file for future exams. All Alberta Health Cards are checked at the time of the exam, to ensure that they are still valid. Alberta Health Care does not cover routine eye exams for adults ages 19-64.

Insurance Coverage for Children (Ages 0-18)

Children are covered under Alberta Health Care for an annual eye health examination. Additional eye exams may be covered if they are considered warranted by an optometrist, or in the case of eye infections or ocular emergencies. If additional follow-up care is required or a patient chooses to receive a second opinion they may not be covered under Alberta Health Care, and will be responsible for payment of services received. It is the responsibility of the patient to accurately inform us as to the date of their last eye health examination.

Insurance Coverage for Seniors (Ages 65+)

Seniors are covered under Alberta Health Care for an annual eye health examination. Additional eye exams may be covered if they are considered warranted by an optometrist, or in the case of eye infections, chronic eye diseases and ocular emergencies. If additional follow-up care is required or a patient chooses to receive a second opinion they may not be covered under AHC, and will be responsible for payment of services received. It is the responsibility of the patient to accurately inform us as to the date of their last eye health examination.

Insurance Coverage for Adults (Ages 19-64)

Adults between the ages of 19-64 are not covered under Alberta Health care for routine eye health exams. However, adults are covered for ocular emergency exmas, along with ocular health evaluations for some chronic medical conditions, such as diabetes and high blood pressure. Its important to realize that these exams do not include a prescription for eyeglasses, sunglasses or contact lenses and additional fees may apply for these services. If for any reason, we are unable to receiving funding under Alberta Health Care for your visit, the patient will be held responsible for any outstanding balances.

Private Insurance for Eye Exams

*Alberta Blue Cross

We are now able to direct bill Alberta Blue Cross for routine comprehensive eye exams. Simply show us your Alberta Blue Cross card, and we will take care of the rest. Unfortunately the system does not allow us to pre-determine the amount of coverage you have. Not all plans are the same, and the amount paid varies. Typically Alberta Blue Cross vision care plans cover $0, $50.00 or the full $80.00, depending on the coverage you've chosen in your insurance. Patients are responsible for any remaining balances at the time of the exam. 

*Alberta School Employee Benefit Plan

The Alberta School Employee Benefit Plan is administered by Alberta Blue Cross. As such, Kingsway Optometry is able to direct bill the teachers insurance plan for most services within our office.

*Other Insurance Plans - Manulife, GreatWestLife, etc. 

We are unable to process direct payments from these insurance companies at this time, as these insurance providers do not offer direct payment options. These companies all work on a reimbursement for services. We will provide you with 2 copies of your receipt at the time of your exam so as to make this process as easy as possible. If you have insurance forms that need to be filled in or signed by the optometrist, please bring them to your eye exam. There is no additional charge for filling out insurance forms. 

Government Programs for Eye Exams & Glasses

Indian Affairs (IA) 

Please notify our staff at the time of booking if you plan on using your Indian Affairs (IA) benefits card. Currently, we must fax in for authorization which can take upwards of 1 business day to receive confirmation. We are unable to attain authorizations over the weekend, so please call in advance to ensure that we have adequate time to receive an authorization number. Individuals who are in a hurry, are more than welcome to pay the regular examination fee and submit their receipts to Indian Affairs for reimbursement. 

Social Services (SS) 

Please notify our staff at the time of booking your eye exam if you plan on using your Social Services (SS) benefits card. Determining eligibility for an eye exam is relatively quick with this program, as they use an automated phone system. Please keep in mind that we are only obtaining authorization for your eye exam and that it does not guarantee eligibility for eyeglasses.

Assured Income for the Severely Handicapped (ASIH) 

Please notify our staff at the time of booking if you plan on using your Assured Income for the Severely Handicapped (AISH) benefits card. Determining eligibility for an eye examination is relatively quick with this program, as they use an automated phone system. Please keep in mind that we are only obtaining authorization for your eye examination and that it does not guarantee eligibility for eyeglasses.  

 

**IMPORTANT**

Please remember that insurance is considered a method of reimbursing the patient for fees paid to the doctor or to the doctor for services performed on the patient and is not a substitute for payment. Some companies pay fixed allowances for certain procedures and others pay a percentage of the charge. It is your responsibility to pay any deductible amount, co-insurance or any other balance not paid by your insurance. If you have already maximized your benefits for a particular billing cycle under Alberta Health Care, you will be held fully responsible for any outstanding amounts. The Alberta Health Care billing cycle is from July 1 to June 30, please notify our office staff if you have received any eye care between these dates at another eye care clinic, so that we can provide you an accurate estimate for your consultation.
